THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN ACCESS THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.
Wigal & Kluemper Orthodontics is required by law to maintain the privacy and security of your protected health information (PHI). We follow the Health Insurance Portability and Accountability Act (HIPAA), including the February 2026 federal updates, and applicable Ohio law.
To provide, coordinate, or manage your orthodontic care.
To bill and receive payment from you, insurance carriers, or responsible parties.
For quality improvement, staff training, compliance, licensing, audits, and business management.
We may contact you by phone, voicemail, text, email, or mail regarding appointments, treatment, or office information.
When disclosure is required by federal, state, or local law, court order, or public health authority.
We may share PHI with third-party vendors (billing services, software providers, labs, consultants) who are legally required to protect your information.
Federal law provides additional protections for certain substance use disorder (SUD) treatment records. If our practice receives or maintains records subject to 42 CFR Part 2:
We will not use or disclose those records without your written consent unless permitted by federal law.
Redisclosure of such information may be prohibited without additional authorization.
Violations may result in civil or criminal penalties.
These protections apply only if such records are maintained by our practice.
You have the right to inspect and obtain a copy of your health records; request amendments; request restrictions; request confidential communications; receive an accounting of disclosures; and receive a paper copy of this Notice at any time.
We are required to maintain the privacy and security of your PHI; notify you if a breach occurs that may compromise your information; and follow the terms of this Notice. We reserve the right to revise this Notice. Updated versions will be posted in our office and on our website.
If you believe your privacy rights have been violated, you may file a complaint with our Privacy Officer at Wigal & Kluemper Orthodontics. You may also file a complaint with the U.S. Department of Health & Human Services. We will not retaliate against you for filing a complaint.
